REFEREE FORMS

Here are some of the forms that may be required for PSL Referees:  The Team Roster is normally provided by the teams to the referee prior to the game.  The Referee Evaluation is provided by the referees to each team prior to the game.

• PSL Team Roster and Game Report (each team should provide one copy)
• Referee Evaluation (provide to each team prior to game)
• PSL Send-off Form (submit within 24 hours!)
• PSL Ray Rates for 2007/2008
